It’s amazing that you have decided to join my Personal Training Programme, I very much look forward to helping you on your health and fitness journey.
There are a couple of things we need to complete before you get going, firstly for insurance reasons you will see a Par-Q (Physical Activity Readiness) form, this is a short health questionnaire just to ensure you are ready to safely start a training plan with me. Secondly, detailed below is my standard terms and conditions which I kindly request you read, understand and agree to.

Regular physical exercise is one of the best ways to keep healthy. However, exercise does carry small risks to certain groups of people and this questionnaire will determine if you are physically ready to undertake a sensible training plan. If you are aged between 15-69 and have no pre-existing medical conditions, this form will highlight if there is a need for you to consult with your doctor prior to starting exercise.

Personal Training Client Agreement - Policies, Terms and Conditions
By investing and participating in personal training, you are fast tracking your results and I have faith that by working together we can make the changes you want in terms of your health and fitness. However, it does take two to get results. Success requires a level of personal commitment from you as well.
The most important factor in improving health and fitness and achieving results is consistency. Training regularly and turning up for each session is just as important as the quality of the workout. Personal training guarantees you will receive quality instruction and professional service. Our training session is valuable and I ask that you respect this by giving me notice of at least 24 hours prior to any designated session if you are going to be unable to attend. If session cancellation has been served in accordance with this policy, I will endeavour to re-schedule the session. Any re-scheduled session will need to be attended within the same 4 week period as the cancellation and will be subject to availability.
Your monthly payment represents the support and preparation outside of sessions as well as the sessions themselves. Payment is due in advance of each 4 week block and all purchased sessions will need to be attended during this period. I will endeavour to accommodate changes in schedule where possible however if you have planned absences during this time please let me know ASAP to agree alternative dates or an extension.
Your payment represents your investment in your wellbeing and covers more than the exercise sessions themselves. Our focus is on working together to get the results you are after, I will therefore be providing appropriate nutrition information, support, check in opportunities, workout suggestions for you to do outside of our sessions and suggested additional activity levels to maximise your results. It is always your choice whether you make use of these options or not.
Injury Rehabilitation Policy
I understand that there are times that you may not be able to participate in the session due to an injury. But this does not always mean you should not train at all. As long as you give me 24 hours’ notice I can plan an alternative session for you to take into consideration your injury. It is important to keep the momentum going!
I request that you only use my mobile number for urgent communications relating to sessions and use my email address (info@StudioSE9.com) for all other communications. Wherever possible I will respond to any communications within 24 hours.
As with any professional business, I am not happy unless you are. If for some reason you are not happy with my service can you please let me know so I can endeavour to provide you with the highest quality of service and rectify any problems as soon as possible.
In order to get the best results possible, I recommend you make full use of the check in opportunities provided and my knowledge. If at any point you would like to know the reasoning behind any of the programming or suggestions I make please feel free to ask, I’m always happy to provide explanations.
The Coach promises to support and advise the Client to the best of their knowledge, taking into account the Client’s preferences and circumstances wherever appropriate. The Client understands that certain elements of personal training can be physically demanding. As a condition of their enrolment, the Client accepts full and complete responsibility for their own participation in the sessions and for their physical and emotional wellbeing and the attainment the agreed goals.
